USMLE Step 1 ABO Groups

By the end of this USMLE Step 1 lesson, you will be able to answer the following questions:

  1. Erythroblastosis fetalis – associated with ABO incompatibility vs. Rh incompatibility?  Why does this make sense?
  2. How prevent Hemolytic Disease of the Newborn (HDN)/erythroblastosis fetalis in second child in Rh-negative female?  Why?
  3. What are the major benefits of a conjugate vaccine?  Why?
  4. If a patient has A+ RBCs, what antibodies would they produce?  Why?

What is the function of B-cells?

To produce antibodies

What generally do helper T-cells (CD4+) do?

Help coordinate the immune response – they are the “quarterback” of the immune system.

What kinds of macromolecule(s) can you generally activate T-cells against? Why does this make sense?

ONLY protein (NOT carbohydrates or lipids)

MHC molecules exhibit protein

What kinds of macromolecule(s) can you generally activate B-cells against?

Protein, carbohydrates, AND fats

What is isotype switching? What is required?

B-cells originally produce IgM. Isotype switching is where B-cells are induced to change the Fc region of the antibody (the antigen-recognition portion of the antibody is conserved) → IgG, IgA, IgE, or IgD.

Helper T-cells MUST be activated, and then co-activate B-cells in order to isotype switch

If I activate a B-cell against a carbohydrate, what antibody isotypes can I produce? Why does this make sense?

IgM

If I activate a B-cell against a carbohydrate, can I produce IgG?

NO. Recall that you MUST co-activate a CD4+ T-cell in order to have isotype switching to make IgG.

Recall, also, that T-cells MUST be activated against protein, and T-cells are necessary to induce isotype switching in B-cells from IgM → IgG, etc.

What RBC antigens are regularly tested for in pregnancy?

A + B antigens (part of the ABO nomenclature), and Rh antigens (the “positive” in Rh-positive).

Why are they such a concern?

Our bodies will make antibodies against any foreign antigen. As such, if we are exposed to RBCs that contains antigens on its surface that are not present on our RBCs, we could mount an immune response against the antigens, and thus the RBCs they coat

If I had antigen A on my RBCs, but no Rh, what would you call me?

A-negative (A antigen is present, but Rh is absent)

What are A or B antigens made of (part of the ABO antigen system) on the RBC?

Carbohydrates (sugars)

What kinds of antibodies can you make against sugars? Why?

IgM

In order to have isotype switching

What is the Rh antigen made out of on the RBC?

Protein

What kinds of antibodies can you make against Rh?

IgG (can undergo T-cell dependent activation of B-cells)

Which antibodies can cross the placenta?

IgG

What is passive immunization vs. active immunization?

Give pre-made ANTIBODIES administered to patient

Vs.

Induce immunity IN patient by giving some sort of ANTIGEN

Erythroblastosis fetalis – what is it?

Antibodies cross the placenta, and attack the RBCs of the fetus. In response the fetus will try and increase RBC production, causing erythroblastosis (recall that an erythroblast is an early RBC)

Basically hemolytic disease of the newborn (antibodies causing hemolysis), but while in utero (technically not hemolytic disease of the newborn, since the baby hasn’t been born yet)

What is more concerning – Rh-negative mother w/ Rh-positive fetus, or an Rh-positive mother w/ Rh-negative fetus? Why?

If negative for the Rh antigen, the mother will develop antibodies against it if exposed to the baby’s Rh antigen.

The fetus has an immature immune system, so would not pass antibodies to the mother

Recall that babies rely on maternal antibodies FOR THE FIRST 6 MONTHS OF LIFE. Thus, if they are B-cell deficient (i.e. btk deficiency) immunodeficient, IT WILL OFTEN PRESENT AROUND 6 MONTHS OF AGE (this is a key Step 1 fact)

When is the mother most likely to become sensitized (produce antibodies against) Rh antigen? Why does this make sense?

During the birth process, which is very traumatic, and during which many blood vessels/tissue will rupture, causing fetal Rh antigens, including Rh antigens, to spill into the blood of the mother. It is at this time that Rh sensitization is most likely to occur.

Let’s say you have antibody-coated RBCs in a test tube. If I added an antibody that will bind to antibodies, what would you see happen?

The anti-antibody antibody (that’s a mouthful; it’s easier to say, “antihuman globulin,” which is the technical term used) would bind to the antibodies on the RBCs, causing them to AGGREGATE.

Direct vs. indirect Coombs tests – what is the basis? What are you actually testing for?

Direct: Anti-Ig antibody (Coombs reagent) added to patient’s RBCs; agglutinate if RBCs are coated with Ig

Indirect: Normal RBCs added to patient’s serum agglutinate if serum has anti-RBC surface Ig, following addition of Coombs (antihuman globulin) reagent

Let’s switch gears for a second, and tie in what appears to be completely unrelated (at least on the surface).

What is a bacterial capsule made out of? What bacterium is the exception, and what is its capsule made out of?

Polysaccharides (sugars)

Bacillus anthracus – capsule made out of amino acid (glutamate)

What is the antigen used in vaccines against encapsulated organisms?

Made out of capsular (carbohydrate) antigen

What is the problem with using ONLY a capsular antigen in a vaccine?

If it is ONLY a carbohydrate, you canNOT activate T-cells, meaning NO isotype switching, NO somatic hypermutation, and NO memory B-cells.

What is a conjugate vaccine?

Conjugate vaccine = typically a vaccine of a carbohydrate (e.g. capsular) antigen CONJUGATED with a highly immunogenic protein (e.g. pertussis toxin).

Erythroblastosis fetalis - associated with ABO incompatibility vs. Rh incompatibility? Why does this make sense?

Seen in newborns due to Rh or other blood antigen incompatibility → mother’s antibodies cross placenta and attack fetal RBCs

Recall that you can make IgG against protein antigens, and since Rh is a protein antigen, you can make IgG, which can cross the placenta. ABO is a carbohydrate antigen, so you CANNOT make IgG against it, since T-cells cannot be activated against it to cause isotype switching in the B-cell.

How prevent HDN/erythroblastosis fetalis in second child in Rh-negative female? Why?

Basically, try and eliminate fetal antigens that pass to the mother.

Rho-Gam – anti-Rh antibodies that you inject into the mother, which eliminate the Rh antigen from the mother’s blood stream BEFORE she can develop her own antibodies against it.

What are the major benefits of a conjugate vaccine? Why?

Immunogenic protein allows for T-cell activation, which allows it to be activated, and thus co-activating B-cells to allow for isotype switching, somatic hypermutation, and memory B-cell formation.

If a patient has A+ RBCs, what antibodies would they produce? Why?

Anti-B antibodies.

The patient has A and Rh antigens, so will not develop antibodies against it (this is known as “self-tolerance,” where you do not develop an immune response against your own proteins). However, the patient does not have the B antigen, so will produce antibodies against it.
